Terms and Conditions
Your Pre-Authorized (PAD) Agreement
You agree to have your account (detailed above) withdrawal take place on the same day as your payday
You understand that the total amount withdrawn from your account may change due to NSF, closed bank account or changes to your benefits coverage. You have waived your right to receive pre-notification of the amount of the PAD and agreed that you do not require advance notice of the amount of PADs before the debit is processed.
The monthly premium debit amount will be deductions based on cost of benefit coverages selected, including sales taxes if applicable.
You have certain recourse rights if any debit does not comply with this agreement. For example, you have the right to receive reimbursement for any debit that is not authorized or is not consistent with this PAD Agreement. To obtain more information on your recourse rights, contact your financial institution or visit www.cdnpay.ca.
Upon completing your authorization on Walmart benefits website site it will remain in effect until your benefit plan coverage has terminated, or until a withdrawal cannot be processed, whichever is earliest.
Cancellation of Debit Withdrawals
To cancel debit withdrawals, contact the Walmart Benefits Centre at 1-800-367-9883. Representatives are available between 8:00 a.m. and 8:00 p.m., Eastern Time, Monday through Friday. If your request is received:
i) At least five business days before the due date, the cancellation will be effective as of the next payment.
ii) Less than five business days before the due date, the cancellation may not take effect until the next billing period.

If this form is for your initial enrolment and is received before bills are produced, it will be effective for the next payment, which is the same day as your payday. If the form is received after bills are produced, it will be effective for the next billing period.
If your request to change your current banking information has been received within three business days before the due date, your request may not take effect until the next billing period.
This authorization will remain in effect until Walmart benefits center has received notification from you of its termination in such time and in such manner as to afford the Walmart benefits center a reasonable opportunity to act on it, until your benefit plan coverage has terminated, or until a withdrawal cannot be processed, whichever is earliest.
As part of this agreement, you hereby authorize Walmart, or its duly authorized representatives, and the financial institution below to initiate personal pre-authorized debit (PAD) withdrawals from your chequing/savings account for your benefit plan premiums.
